How do you actually get your cases into Typhon/Medatrax?

Hey all, not an SRNA here but curious how this works in practice.

1. When you sit down to log, what are you working from? Notes app, something you scribbled, memory?
2. Which fields do you end up guessing at?
3. Phone or laptop?
