28 @ 389 ng/dL, stupid or right to consider TRT?
28M – 389 ng/dL testosterone after 6+ months in a large calorie deficit. Looking for opinions before considering TRT.
For context I’m a 28-year-old male, 5’8”, physically active and lift 4-8 days a week.
Over the past year and a bit I’ve lost a good chunk of weight, never been obese but I was heavily bulked I would say. I went from roughly 225 lbs to 160lbs and lean.
April 2025 I went into a heavy ish deficit of 600 calories then extending to 900 calories (obviously during this time my maintenance will have dropped considerably with my weight but from my starting maintenance that is where I was at. I did do 3 months at 2500 calories from Dec-Feb just to give my body a break from cutting but then returned to a 700 calorie deficit.
Total Testosterone as of this week is 13.5 nmol/L (389 ng/dL)
Previous result during the 3 months at maintenance was 16.2 nmol/L (467 ng/dL)
LH: 1.6 IU/L
FSH: 1.3 IU/L
I have never had total T checked which I realised kind of makes this obsolete, I will be including that in my next blood panel whenever I choose to do one.
Everything else was largely within range, although I do have a family history of heart disease and high cholesterol, so that’s something I’m keeping in mind if TRT ever becomes a consideration.
My question is whether this looks more like functional suppression from prolonged calorie restriction, or whether these numbers are low enough that I should seriously be considering TRT.
Part of me thinks it makes sense to spend a longer time eating at maintenance or a slight surplus, recovering properly, sleeping well and then repeating bloodwork before making any long-term decisions.
On the other hand, I’m wondering if 389 ng/dL at 28 is low enough that it’s unlikely to improve significantly.
Has anyone here been in a similar situation after a long cut? If so:
How much did your testosterone recover after returning to maintenance?
How long did it take?
At what point would you consider TRT instead of continuing to wait?
Interested to hear both personal experiences and any thoughts on my bloodwork.
