Made a mistake regarding Foley catheter.

RANT

I had a patient who was an RRT from the floor. They had an AKI, rhabdomyolysis and were retaining urine. My nightshift provider gave the verbal okay for a foley. She said she would put the orders in. I put the foley in okay no problem. Night shift provider knew about it. I didn't check for the order in the chart. I trust this provider to put in the orders. My manager emails me that day regarding the foley. Turns out there was no order for it. Lesson learned make sure all the appropriate orders are in the chart.
