How does the emgs health form work?

Hi guys, i'm applying for my eVAL and it says to declare whether "I am free from the following diseases/conditions", so would it be correct to tick "YES" for a psychiatric condition like anxiety/depression I had in the past, which I'm now free from? Or should I declare "YES" for any condition i've ever had?
