Documents for Offline Reporting

The indemnity bond is to be taken on a 100/- non-judicial stamp paper and be notarised but I had a few other doubts like are all details in the blanks to be hand written or should the be printed….
And what about the date and place like should it be on the date of physical reporting or if I get it done today so today’s date will be mentioned… Place should be Jaipur right??
I know this might sound stupid but I am really confused and it will be a great help if any senior or any fellow student who knows what is to be done can guide me…
