Need to confess

I want to confess one thing which i don’t know how you all will take but i really need your help.
Now i am 27(M)
I’m doing masturbation from last 15-17 years and with porn around 12-13 years.
Its not like i never tried to quit it but every time I tried i relapsed.
The maximum time i being controlled is 20-25 days but now i am seeing the side effects of it
Earlier i can involve with my girlfriend 4 times in a days but now just after 1 time i did not feel like to do.
My self confidence, memory is all gone.

If someone from this community has experienced the same.
I need advice and suggestion what should i do to get back to normal.
Or it’s too late !!
