Question About Diaphragmatic Endo Surgery

Diagnostic Journey Questions

After years of unexplained infertility, I finally had stage 3 cul de sac endo confirmed via diagnostic laparoscopy earlier this year. An MRI was inconclusive for DIE. I was referred to a MIGS endo excision specialist in my city.

I’ve had a symptom for years (intermittent difficulty breathing that was diagnosed as chronic panic attacks when I was in college) that based on a few of the experiences I’ve read online seems to line up with diaphragmatic endo. My endo-focused OBGYN said that she checked and didn’t see anything around my diaphragm but to definitely bring it up with the specialist although in her experience, that wasn’t a symptom she’d heard associated with diaphragmatic endo.

So, I brought it up to the NP with the specialist’s office in my initial evaluation. She stated two things that seemed a little off: 1.) my specialist and his surgical assistant will be the only 2 medical practitioners in the room (there will not be a colorectal surgeon or any other specialists present), and 2.) that my specialist will not operate on my diaphragm- that there’s really only one surgeon in the US who does that surgery and that if I do have that, it will definitely be a separate procedure that I’ll have to seek out on my own.

I’ve seen videos from folks at the ESSI group and Dr. Liu on Insta that suggest that it’s a red flag to be told you may need multiple surgeries or to not have every specialist in the room. Is it really? Or are they just competing for patients? I’m now 3months out from surgery and I desperately do not want to “start over” on the waitlist with another surgeon.

For additional context, I don’t have any pain- I’m pretty asymptomatic. My only goal is to be able to finally have children. I started “trying” at 26. I’m now 33. I’d wanted a big family. I’ve never had a positive pregnancy test. I don’t want to go through risky pregnancies in my later 30s. I just want to get this excised and hopefully be pregnant and be able to have back to back pregnancies and be done having kids by 36 or 37 at the latest. If we’re talking about me waiting on some list for another year, that dream is basically dead. I’m very torn on how to proceed.
