Medical form confusion

Recruiting

Hoping the lovely medics on here could answer this. During my Medical II, I was given an MH form to fill out as I have very mild ADHD. I couldn't initially get a hold of my family doctor, so I resorted to getting a walk-in doctor to fill it out as I had supporting documentation, then proceeded to submit it. A few days later, my family doctor got back to me and sent me the completed form, which I then forwarded to the medics who were handling my file as I was initially asked to have the prescribing clinician fill it out. They acknowledged receipt of both of them, telling me they've been added to my file before being sent off that very day.

Now, here are my questions:

  1. Could the fact that I've submitted a duplicate of the form filled in by two doctors cause issues with the RMO? For the record, both doctors said the same thing (i.e mild ADHD, occasionally take medication as needed, fully able to handle stress, not a danger to myself or others etc etc).

  2. The form my family doctor filled out lists my old medication (Concerta vs Vyvanse) whereas the one the walk-in doctor filled has the accurate medication. Could this discrepancy cause me issues? I have my prescription so proving that I'm on Vyvanse and not Concerta would be a cake walk.

I understand that only the RMO can make a decision, not worried about that, more-so worried about the forms themselves causing confusion and further delaying the process.

Thanks!
