Do I give up on the relationship or keep fighting the mental health battle ?

We have been together for over 13 years and we had a great first half of the relationship. Things started going downhill after covid hit. He started not wanting to go out anymore (even when things opened back up) would sleep longer through the day, he stopped showering and taking care of his basic hygiene unless he had to meet up with work (he works from home). He also had back pain from a car accident. Through this we moved closer to his work (hoping to get him in the office more) making my commute long and I spend hours in the car each day.  I do all the house work, grocery shopping, cooking ect. Well this past week I have been having my own health issues making is painful to do a lot of the chores. I ordered groceries to be delivered cleaned the house did the bare minimum but what needed to be done all while my boyfriend slept (he had been sleeping at this point for 3 days getting up for food and basics and not working).

When he finally wakes up and comes into the kitchen I had to hear about how I didn’t get the right stuff for him to eat, I should of gotten more of certain items and I should of known he needed more and then when it came time for dinner I had to put his food together because he couldnt figure out how to heat up food. This ended when him telling me how I did it wrong and he would of done it better. I lost it on him and it wasnt nice the words I used. I just dont have any more empathy left, this is an on going batter where I yell and cry and ask him to get help and make changes, and he does for a week or so and then we revert back to this person. Do I keep staying and trying to help? Or am I just holding out hope for the person I use to love and should give up?
