Hospice Documentation

Hi everyone! I hope you're all having a great day.

I have a question for those of you working in hospice. How long are your routine RN visit notes?

I'm trying to improve our documentation process and was curious what other agencies are doing.

I feel like our nurses write very long notes, but they don't always clearly demonstrate disease progression. During ADRs, I've noticed that it's not about how much you write—it's whether the documentation supports continued hospice eligibility.

Do you keep your routine visit notes relatively concise and focus on changes since the last visit, or do you document everything every visit?

I'd also love to know:

  • About how long are your routine RN notes?
  • What do you make sure to include to support eligibility and recertification?
  • Has anyone changed their documentation style after going through ADRs?

I appreciate any insight you're willing to share. Thanks in advance!
