Hospital Choice and Quality Where UK PMI Policyholders Go for Treatment

Hi all,

One of the most common topics that comes up here is hospital choice. It's a key reason many of us get private cover in the first place, but how it actually works can be a bit opaque. I was reviewing some material on this and thought a summary might be useful for the community.

Essentially, your choice of hospital isn't unlimited; it's dictated by the 'hospital list' attached to your policy. Insurers create these lists to manage costs, and the tier you're on is one of the biggest drivers of your premium.

Generally, they fall into three main categories:

  • Reduced or Guided Lists: This is the most budget-friendly tier. Your insurer provides a smaller, curated list of hospitals they've found to be cost-effective and high-quality. Some policies also use a 'guided consultant' model, where they'll recommend specific specialists to keep costs down. It’s a trade-off between choice and premium.

  • Standard or Core Lists: This is the most common option and what most policies default to. It offers a comprehensive network of private hospitals across the UK, including the big groups like Spire, Nuffield Health, and Circle. For the vast majority of people living outside Central London, this list is more than adequate.

  • Extended or Premium Lists: This is the top-tier option with the highest premium. It includes all the hospitals on the standard list plus the most prestigious and expensive facilities, which are mostly concentrated in Central London (think Harley Street area, The London Clinic, etc.). This is really for those who want maximum choice and are willing to pay a significant premium for it.

The main takeaway is that there's little point paying for an extended list with top London hospitals if you live in another part of the country and are unlikely to travel for treatment. Matching the list to your actual geographic needs and expectations is key to not overpaying.

Full disclosure: these insights are from a guide I work on.

For those with a policy, have you ever found your hospital list to be a limiting factor when you needed treatment, or has the standard list always been sufficient for your needs?
