Filing a Federal Safety Defect Complaint with NHTSA (Office of Defects Investigation) —Need YOUR Stories & Incidents
I am in the process of submitting a formal safety defect report to the National Highway Traffic Safety Administration (NHTSA) through their Office of Defects Investigation (ODI) regarding severe equipment hazards and product failures of the Intoxalock device.
I already have a large collection of documented review data, but I want to make this federal filing as comprehensive and undeniable as possible—and I need your help.
I am looking for specific evidence & experiences regarding:
- Mouthpiece Failures: Mouthpieces popping or falling off while driving at highway speeds during a rolling retest.
- Physical & Health Hazards: Drivers experiencing extreme dizziness, hyperventilation, or lightheadedness due to required blow/hum resistance.
- Device Non-Acceptance & False Retests: Random retest failures or refusal to accept valid blows while actively operating a vehicle.
- Extreme Weather Stranding: Getting stranded in extreme freezing cold or summer heat—especially if you had children, family, or dependents in the vehicle with you.
- Any Direct Device Defect: Any instance where the hardware or software caused an active road hazard, loss of vehicle control, or dangerous situation.
How Your Comments Will Be Used:
If you leave a comment below detailing your experience, I will copy and paste your words exactly as written and attach your statement directly into my formal report dossier for NHTSA investigators. I will report them on my LinkedIn page where I already have established connections with the very top leadership of NHSTA.
File Your Own Individual Complaint (Highly Recommended):
Federal regulators track defect patterns by the volume of individual consumer reports. If you have experienced any of these hazards, please file your own report directly with NHTSA as well. It only takes a few minutes:
