Setting Difficulty ?

Hi all,
New to the game, level 8.
Please can someone advise about setting difficulty for main missions for First time, is it alway in Story Mode difficulty for first completion, and the you can change difficulty only when replaying ?
Is there no global difficulty setting for the whole game please ?
