I feel sick. Can anyone try to tell me what I have?

I am 19 I am 5'10. I weight 230. I recently started drinking only water and eating what I eat. I take Focalin Xr 40mg, Fluoxetine 80mg, Risperidone 3mg, Dexmethyphenidate 10 mg, and guanfacine 3mg. I've been taking my focalin xr at 5-7pm at night instead of the morning. I have a weird feeling by my heart. I have a weird feeling in my chest. I went to the doctors to see if I have internal bleeding and once before when they said I had a panic attack. The panic attack felt like if I couldn't breath for 3 seconds then it came back and my heart was beating really fast. The feeling before the panic attack comes back sometimes but the breathing thing doesn't happen. I had black in my stool 3 times but it wasn't fully black only pieces. I feel out of it. I didn't throw up but I almost threw up once. I went to the hospital to see if I have internal bleeding and they pressed on my chest and stomach and I didn't have it. I have a little trouble breathing.
