How much of a client's actual week do you hear about?

Curious how much real detail people feel like they're getting about a client's week, versus a compressed "pretty good, I guess" summary reconstructed in the first five minutes of session.

Does anyone have a system for clients to log something between sessions that you can actually read before the next one, rather than relying on memory? Curious what's worked, regardless of tool.

For context, this is part of what Relvema's companion app tries to do — give clients a place to log a reflection between sessions that you can see ahead of time. But mostly interested in how people handle this today.
