Palate expansion alone vs palate expansion + djs for tongue discomfort and posture
I have a narrow palate (and crossbite) that my ortho says needs to be expanded by about 7mm with either MARPE or SARPE (he has given both options). I’m a 34 year old woman. No previous ortho work. Treatment would be to help my breathing and tmj and is not just for aesthetics, although that is definitely a factor. Ortho said I might also want to consider a septoplasty for my deviated septum during/after treatment but the palatial expansion alone should help a lot.
I brought up my recessed lower jaw, feeling like I had tongue thrust, and my forward head posture, and queried the option of double jaw surgery to advance top and bottom jaws forward. He said my airway did not look severely constricted, and my lower jaw had only a small amount of recession, so double jaw surgery would be more for aesthetic reasons, and that palatial expansion alone should be sufficient for improving my airway, tongue positioning and head posture.
I feel that my palate not only feels too narrow for my tongue, but also a that the tip of my tongue is pushing up against my front top and bottom teeth as if there’s not enough room front to back. Sometimes I feel my tongue between my top and bottom front teeth. But I’m wondering if maybe I just don’t know how to position my tongue correctly because of my narrow palate, and if my tongue would naturally sit further back in my mouth after expansion.
I’m curious if anyone else had this and subsequently gone through palate expansion alone (with either sarpe or marpe), and if so, did discomfort in this sagittal plane resolve? I’m concerned that I will end up with an open bite or continued tongue discomfort, relapse, etc with palate expansion only vs palate expansion plus maxillomandibular advancement surgery.
I am going to follow up with my ortho again and also bring it up with the surgeon I’m being referred to to discuss sarpe anyway, but just curious about others’ experiences.
