DCIS + Lumpectomy: Has anyone had a successful re-excision after an oncoplastic closure/ tissue rearrangement?
Hi,
Greatful for this resource- let me know if any of this resinates with you or someone you know.
41F with DCIS. I recently had a SAVI Scout–localized lumpectomy with oncoplastic closure/tissue rearrangement. I also have subpectoral cosmetic implants.
My final surgical pathology:
7 specimens were removed total
*Only 1 of the 7 contained DCIS
*Original lumpectomy specimen containing the SAVI Scout: negative for carcinoma
*The only positive specimen was an additional anterior-medial margin
*It contained 3.8 mm of high-grade DCIS
*All final margins are technically negative, but the anterior and medial margins are <1 mm
*No invasive cancer — pure DCIS / pTis
*ER/PR 91–100% strongly positive
*Plastic surgeon performed an oncoplastic closure with approximately 40 cm² of tissue rearrangement/advancement to fill the lumpectomy defect
*SAVI Scout and biopsy clips were removed during the original surgery
->I’m now deciding between leaving the negative-but-close margins and treating with radiation versus potentially re-excising the anterior-medial area. I meet with my doctor tomorrow so this is really just to gather feedback from the community.
->Has anyone successfully had a targeted re-excision after the SAVI Scout/biopsy clips were already removed AND after oncoplastic tissue rearrangement? Was your surgeon still able to accurately identify the original margin? Did plastics have to redo the closure, and how was your cosmetic result afterward?
Big thanks, stay health and positive, sending hugs to you all....
