Incorrect information on a C&P

I just requested records of my C&P exams so I can submit them with a warrant packet. I got my C&P back and it states I had a major surgery (colon resection). I’ve never had this, and no where in my military medical records does it state I had this procedure. I didn’t go to a C&P exam for this, someone just reviewed my records and I received a rating. (I’ll be honest I may have no-showed for the C&P, but I don’t recall 100%)

How exactly do I go about getting this error corrected? I CANNOT submit something with my warrant packet that is not correct. I did some research, my VA rating would have been much higher if in fact I did have this surgery ; the VA rating is appropriate for the condition so I’m not sure where this went a miss.

Desperately needing assistance on how to correct this error. My packet is due in 5 weeks.
