Cream and/or insert

I started on the patch, pill insert combo, and just recently asked my N.P if I could use the cream as well, she asked why and I said, although I can appreciate what the pill insert does, I am in fear of losing my clitoral hood as I have already lost my labia minora. (She laughed when I called myself a realistic Barbie because I now only have a cup) she did say using both is not necessary but she never said the insert would help my lady bits.

So my Question is, is anyone using both? I plan on just using the cream literally just on the hood and what I have left of my minora
