Everest Institute Grand Rapids closed — need help with Massachusetts LPN licensure form
I am posting on behalf of a friend.
My friend graduated from Everest Institute in Grand Rapids, Michigan, and the school closed in 2015. She applied for a nursing license in Massachusetts, sent her official transcript to the Massachusetts Board of Nursing, but Massachusetts also requires a Certificate of completion signed by the nursing school. This is where she has had peoblems. The custodians of the school records say they don't know anything about that form.
She does not know who is authorized to complete and sign this form. Every phone call has been fruitless.
Has anyone here graduated from a nursing school that later closed, especially Everest Institute, and gone through licensure in Massachusetts or another state?
Who did you contact to have the Certificate of Graduation completed? Was it the custodian of the school's records, the Michigan Board of Nursing, the Department of Education, or someone else? You would think the custodians who mailed her transcript to MA would know, but ....
Any advice or would be greatly appreciated. Thank you.
