What do I bring if kukuha ng PSA Birth certificate and NSO Birth certificate lang meron ako?

Hi! I have an NSO Birth certificate and I need to get a PSA Birth certificate. What else should I bring aside from my NSO birth certificate? And how much should I prepare. Would it be preferable to go in person or to request it via e-mail? Thank you!
