LOA question

Does cirrusmd fill out leave of absence paperwork for you if you need to request a medical leave? I don’t know if I’ll be able to get into my primary care doctor today and so I’m trying to see if I go through cirrusmd if they’ll be able to help me. I’m trying not to call in and have points against me but I’m very positive I have food poisoning and the way things are looking I don’t think I’ll be able to make it in.
