Bad situation work - need advice

Please help me out. I am an engineer with a dysregulated nervous system. Official diagnosis from a psychartist is adjustment disorder. In late 2025 I had a ton of work stress got sick. Not the fake it kind of sick, the real kind. For the most part I've improved for the better, the things that linger are trouble staying asleep and ability to handle stress. I also have alot of sleep deprivation from the last few months too. The thought of returning to my old job terrifies me. I was being pressed to approve garbage that was not properly checked and it's high volume and safety sensitive. I also have another job opportunity but I'm not sure I can handle it. I'd be on probation at a new job without the possibility of a gradual return to work or accommodations. I'm truly afraid of return to my current job, they are so reckless in my opinion. This is a big government company to boot. What should I do? Take a new job while not 100% or go to my existing job? My doctors are writing a letter to tell my current workplace HR not to put me back in the same department. Not sure if they will listen. But if I'm returned to the same old situation i think it will negatively affect me.
