Anyone with SIBO/IMO have mostly upper GI symptoms like burping, epigastric burning, and slow digestion?

I’m curious if anyone here has had a similar presentation, because my symptoms seem much more upper GI / small intestine focused than the classic severe bloating/diarrhea picture.

My main symptoms are:

  • Frequent burping, especially 30–60 minutes after eating
  • Burps sometimes feel unusually warm/hot
  • Burning/discomfort in the epigastric area and underneath my ribs
  • Feeling like digestion is sluggish or food is just sitting there
  • Nausea at times
  • Reflux/regurgitation sensations
  • Gas/pressure underneath my ribs
  • Symptoms generally get progressively worse during the day, while mornings are often my best time
  • Smaller/lower-fat meals seem easier
  • Carbs/fermentable foods sometimes seem to make things worse
  • Brain fog at times
  • Significant difficulty maintaining/gaining weight

Interestingly, I usually have a bowel movement every morning and don’t have dramatic diarrhea or visible bloating. An abdominal X-ray did show a moderate stool burden, though.

I’ve had a pretty extensive GI workup: multiple EGDs/biopsies, colonoscopy, imaging, thyroid testing, and a gastric emptying study. The gastric emptying study was normal, and my most recent EGD/colonoscopy and biopsies were essentially reassuring although earlier this year I did show chronic gastritis, duodenitis and esophagitis . H. pylori has also been negative.

My doctors have discussed functional dyspepsia but the noritriptyline 10mg didn't really help, but I’m interested in investigatingSIBO/IMO or abnormal small-intestinal motility/MMC function, especially given the burping, gas, meal-related symptoms, and stool burden.

Has anyone here had SIBO or IMO with a normal gastric emptying study and relatively normal bowel movements, where the main symptoms were burping, epigastric burning/pressure, reflux, and a sluggish digestion feeling?

If so, what did your breath test show (hydrogen vs methane), and did treating SIBO/IMO or addressing motility actually improve the upper GI symptoms?

Also following the meal spacing 4-5 hrs has been helping a least slightly.
