Hashimoto’s causing idiopathic peripheral nerve hyperexcitability
Hello everyone, I’m a 36 male who presented symptoms back in March 2025 of just feeling off feeling like I needed a vitamin. I went to rheumatologist who found my TPO was high and tsh was borderline normal high. Was told to see an Endo or my primary care about a Hashimoto’s diagnosis and have them track my TSH. A few months later, my PCP gave me levothyroxine 50 MCG’s. And I still felt off this was like July 2025 I started to get the stiffness feelings in my legs and my arms. By August 2025, I started getting muscle fasciculations and weakness in every limb when I saw a neurologist he said this is crazy because you’re talking about every limb and this is not like neuron degenerative because of that. Waited until October 2025 and received an EMG that just showed fasciculations in my legs. I was signed out of work because of how fatigue I would get I had no clinical weakness had another EMG done around 1 November same thing, but no fasciculations now in the bottom extremities. They ruled out any motor neuron disease at this point. I had Lyme disease in the past. I went down that route for a little bit and nothing really helped in that realm. Next, I found that I had a positive genetic marker for. SC4NA but my variant is unknown significance. It’s predicted to be a 5% pathogenic variant. My neurologist thought it was a Hail Mary to try to treat it. I tried sodium channel blockers and had cardiac symptoms.
Now we’re almost a full year into this fatigue that I get which is extreme. I get cramps, muscle fasciculations, or to keep my arm up on the steering wheel legs feel like I’m walking on peg legs. I ended up seeing a Dr. Lauren Elman at the university of Pennsylvania Philadelphia. She is the MDA chair person also used to be the head of the ALS clinic for Penn med as well as a teacher and professor of neurology at the school. We spent 80 minutes together she looked at my symptoms she said with almost a year of symptoms going on and no clinical failures that at this point, she would rule out any motor, neuron disease, most definitely but at the same time she feels as though, since this all seem to start after a Hashimoto‘s diagnosis that she feels as though it’s an idiopathic, peripheral nerve hyperexcitability syndrome that seemed to happen while my TPO’s were super high. I normally get my TSH checked by my primary. It’s normally around 2.5. I’m just wondering if anyone has any insight on this because when I saw my endocrinologist, they didn’t even wanna treat me for the high TPO because my TSH was fluctuating between high and normal and said that TPO only means that I have Hashimoto’s and I’m more susceptible to hypothyroidism. It was my PCP who prescribed the thyroid meds. Just wondering if there’s anything else that I’m missing or if anyone has gone down a similar journey or know a doctor on the East Coast Philadelphia, New Jersey New York area that could possibly help in this situation. I’m tired of the guessing games and I just really wanna figure out why I don’t feel good and why I get these sudden crashes of weakness which tend to happen with more repetitive movement if you read this for thank you for caring and please let me know if you have any insight!
