Normal for NPs to make the rules, volunteer physicians for more work, admit less patients, come and go when they want while physician stay behind to cover them
I work as hospitalist physician. Recently signed as contractor for a group that hires ED and hospitalist docs and NPs. Census is about 60-80 over 2 MDs and 2 NPs. Hospital recently decided that NPs don't care for ICU patients. This way, MDs take call every other day, which can be very time consuming.
We as physicians have to take call, distribute patients (including write on board which patient goes to whom when the ancillary nursing staff decides they worked too hard for the day and to leave early), admit ICU patients regardless of order of admission, and additional responsibilities. Not uncommmon for the physician to have a census of 15-20 while the NPs have 7-10, and rare that the Physician has lesser census than the NP.
To top that, the nurses, who have been hired / reared by NPs and have already expressed explicit loyalty to "their people), are the ones who distribute the patients in the morning. They sometimes take advantage of the lack of supervision and a very passive program director to do what they want. For example, despite being on call and with a higher starting census on a typically busy Monday, they gave me a very sick ICU patient, assigned me as the first to admit with an already pending patient, and since I was the first to admit, I was also the one who admitted the most that day. This is not a one time occurrence but a pattern that is worsening.
Other issues are that the environment is very unprofessional. A lot of noise in the shared hospitalist room, one of the nurses demonstratively burps every few minutes, if one of the NP is in a bad mood they put the phone on speaker and have a fight with the IT guy, they rank physician based on their intelligence calling them stupid etc in my presence, and so on. One of the NPs tried to volunteer putting a cap on nocturnist translating to extra work for the physicians who are already working harder.
One nurse in particular, the charge nurse, is on some power trip and already hinted that I should treat her in a certain manner to be on her good side. She is the one who is responsible for most of the strange patient assignments that indirectly affect patient safety.
