Copy ratings and playcount from files with similar metadata when replacing library?

I'm in the process of replacing a large library of mp3 files with FLAC, and I'd like to keep the original ratings, playcount, and positions within existing playlists.

Can I edit the database to point to the new files if they use the same directory structure and have similar or identical names aside from the extension?

Are there any tools for doing this?
