Crossing wheels on group ride

Some debate after a recent incident in our group rides has come up. When on a group ride, the rider in back puts their front wheel past the rear wheel of the rider ahead. Front rider moves to avoid something, unaware the rider behind is that far up. Wheels catch and the riders go down.

Not necessarily looking for "fault" but what should be done to avoid this type of incident when riding in a group?
