What should I bring to be fitted for a wheelchair?

I’m asking here cause I can’t find another place for disabled ppl in Ontario and also it may be different cause I’m covered by ODSP.

So today I got my doctors note to give to the OT so the appointment is covered, but what else should I bring?

Should I get a note from my physio?

Should I bring my diagnosis paperwork?

Should I have notes from family/friends backing me up on my inability to walk long-term?

I’ve gathered so far that the wheelchair assessment is very different from the rollater assessment, so I have zero idea what I’m doing.
