US visa scheduling profile showing someone else's name and dependent status - India

I’m applying for a U.S. B1/B2 visa in India.

Previously, I applied together with my parents. Their visas were approved, but my visa was refused. I am now trying to apply again independently.

When I log in to the US visa scheduling portal, my profile shows a completely different person's name—not my name and not either of my parents' names. The portal also says that I am currently unable to access the visa portal because I am a dependent.

The email address associated with the account is mine, and I have never used the other person's name or received an OTP related to that person.

Has anyone experienced this issue? Could this be an old group/dependent association or a portal data issue? What is the correct way to get the profile corrected/dissociated?
