Disability forms
Would you fill out disability paperwork if this is the first time you’re seeing the patient, but he has been coming to the clinic and being seen by other providers?
Had a patient with diabetes foot infection and recently had two toes amputated. He has been seeing a podiatrist and his PCP.
I told the manager that the form asks when and how often I’ve been seeing the patient and when I anticipate his recovery. I can’t answer those questions because I’ve never personally seen him before. She says I can answer based on the information and previous visits in his chart. But if I’m not wrong, those questions are specifically asking about my own involvement with the patient and my clinical assessment, right? Am I wrong?
