Subluxated rib (again, over and over)

Hi all, new here. I am 39F and my ribs slip allllll the time. This has been a problem for years. It happens several times per year, usually on the left, but occasionally on the right. When it happens, it thumps, hurts, and then typically kind of clicks when I walk and breathe afterwards. The pain ramps up until day 3 where it is turned all the way up to 100 and hangs out there for a couple of days, and then gradually starts to lessen and finally goes away around 6 weeks after the injury. It sucks so, so badly. I have had an L1 burst fracture, an open finger fracture with nail avulsion, and severe osteoarthritis and a closed lock in my left TMJ that required a joint replacement, so I know ball when it comes to pain, and this lays me out every time.

It is so bad that I totaled my car a couple years ago and broke 3 ribs and had no idea, because I was like "ope, I did my ribs again, bummer," then found out later during a PET scan that they had healing fractures in a seatbelt shape. 🙃

Every time, I end up at urgent care for a Toradol shot when it is at the worst point. Muscle relaxers do not help, I have no idea if opioid painkillers would because I don't like them and this happens too often to venture into using them, and OTC Tylenol/Ibuprofen doesn't touch it. I just have to ice it and try not to move. I am currently taking minoxidil but it's really not touching the pain.

I do know that the rib is actually moving, and not me being dramatic, because this current instance it was actually caused by my friend, and he felt it move where he grabbed me, and then felt it thump back to its spot. Also for what it is worth, this type of thing happens or has happened to MOST of my joints, so it is not really surprising that my ribs joined in.

So I have a few questions:

  1. I am obv not a Dr but from my over googling, it sounds like it could maybe be slipping rib syndrome? I also occasionally do the upper portions of my ribs which I think is costochondritis? From similar mechanisms but near my sternum. I don't feel those thump out of place, but they do usually still click afterwards.

  2. what kind of doctor do I see for evaluation and treatment of rib problems? I have been to several urgent cares, one ER, my PCP, and called orthopedic practices and even a thoracic surgeon once, and no one really knows what to do with me. I know there is really not a lot you can do, but if I do have slipping rib syndrome, the internet says physical therapy may help? And also talks about intercostal nerve blocks, which sound SO MAGICAL right now. The urgent care PA last night gave me a referral to a PM&R doctor, but she wasn't sure if that was the right place for me.

  3. is the pain really because of irritated nerves? And not bc of the cartilage being inflamed after an injury? If so, could gabapentin possibly give me some relief? I have like... A lot LOT of it leftover from my various surgeries, and I don't remember it doing much for them, but then again they weren't really nerve injuries? I just need a little bit of relief and I don't want to have to go back to urgent care again. I promise I am not trying to do drugs/crimes, I am just miserable.

Anyway I would appreciate any general advice... In the moment, I am desperate for a little bit of pain relief, in the long term, I want to be able to both prevent it AND have somewhere established to go for real actual relief when it inevitably happens again.
