New Player What Do?

Hello!

Just picked up the game after years of watching it. Lots of RL experience but know little about Isaac. I browsed the links on the subreddit but there's so much to this game and a few things I still don't get.

1. Is there any way to track my progress/set a road map for unlocking new items (ala Vamp Survivors?) or is it just best to keep playing and see what happens?

2. I see lots of screenshots on here of people with their items visible on their screens along with the major skills. Is this an unlockable feature of the game like item descriptions or is it mods?

3. I've played like 20 games, killed the heart 4-5 times, what do I do now? (I'm hooked)
