Chronic low ferritin, norethisterone, no diagnosis in sight

Rant / Vent

I've been anemic for most of my adult life, despite a normal diet. My periods are not heavy. I don't soak through pads quickly or anything like that. In fact my flow usually dwindles and starts stopping on the third day, and then there's mostly just spotting for 1-2 more days. My cycle is on short side though (23-24 days).

I have seen so many doctors over the years for the anemia. I'd always just get prescribed iron pills and sent on my way but it eventually comes back. Last year I had ferritin of 5 (and almost inexistent folate too) along with the anemia and was prescribed iron pills to rebuild my stores. Few months later I finally built up the courage to see a gynecologist for my disabling period cramps (i tried before and got dismissed, told that my periods would naturally regulate with age and most women had painful periods too), but as soon as the blood tests she ordered showed severe ferritin deficiency (5.5, apparently it barely improved despite the pills) she blamed it on heavy periods. I explained that my flow seemed normal to me but she was very dismissive. As for the cramps she just looked for uterine cysts, found nothing, and more or less informed me that there was nothing wrong with me. Beyond the pain she had no explanation for the extreme nausea, fatigue and non-stop throwing up that i experience during my periods, unable to even leave my bed or eat (and according to my blood tests i'm not even anemic right now, just ferritin-deficient).

I'm currently on 10mg norethisterone to stop my periods to "allow my iron levels to go up". The side effects have been manageable. After a perfect first month, i had a very sudden mini-period that lasted a couple hours, and i've been spotting and/or very lightly bleeding non-stop since then (some of it looks like fresh blood, some of it older and more brown than red), along with slight uterus cramps everyday, 8-9hours after taking my pill. Nothing even close to what i usually experienced during my periods though. I'm also experiencing bloating in my lower belly. I wake up bloated, i go to sleep bloated. I'll have a toast and egg for breakfast and feel stuffed to the point of discomfort and physically unwell. I'm used to being bloated during my whole cycle except for the days immediately after my period, but it was never so extreme and so visible.

I'm honestly lost as to what to do next, other than looking for a different gynecologist and advocating for myself more. This is a rant for me but if anyone had a similar experience with deficiencies/anemia and ended up diagnosed with endo, or has some insights to offer (things i should bring up or tests i should ask for during my appointment), feel free to share of course.
