Should I get another endoscopy?
33M. About 4 years ago I developed gastritis after drinking too much alcohol. It eventually healed after months of dieting, but I was left with occasional achy/crampy tightness under my ribs, especially during exercise. Later I went on an SNRI for unrelated reasons and that pain basically disappeared.
I’ve since had two follow-up endoscopies (2022 and 2023/24) which were completely normal, including biopsies.
Recently I’ve had a flare of similar symptoms for the first time in years. Just over a week ago I started pantoprazole because I had some mild nausea and a dull lower-right ache. I honestly can’t remember having much of this under-rib discomfort before starting it, but over the following days I began getting the old tight/achy/stitch-like sensations under both ribs and sometimes centrally.
The confusing part is the PPI definitely improves my silent reflux/throat symptoms, but my upper abdominal symptoms almost seem worse on it.
I only took it daily for about 5–7 days and have now stopped. No vomiting, appetite loss, black stools, etc. I can eat normally — it’s mainly the rib/upper abdominal discomfort.
Has anyone had PPIs cause or worsen this kind of bloating/tightness/dyspepsia? Or does this sound more like a gastritis flare vs functional dyspepsia/visceral hypersensitivity?
