Secondary claims

Do you start a new claim for a secondary to something that's been service connected? Or do you have to appeal then add it as the appeal?

I get a lot of virtigo due to my ears being not so great anymore. VA denied hearing loss even with evidence but gave 10% for tinnitus. Between the tinnitus and my migraines I get a lot of virtigo. Appreciate the insight.
