QTS about AHIMA Cancer Registry Operations course / Appendix

Hi everyone! Is anyone here currently taking or has completed the Cancer Registry Operations (CRO) course through AHIMA?
I was going through the course outline and assigned readings, and I noticed that one of the assignments references “Appendix G: Case Completeness Tool.”
I’m a little confused about which book this appendix is supposed to be from. I have the Cancer Registry Management: Principles and Practice for Hospitals and Central Registries, 4th Edition, but the PDF I have doesn’t seem to include Appendix G—or any of the A–Z appendices.
Could someone please clarify where Appendix G (Case Completeness Tool) is located or which resource/book it comes from?
If anyone has access to the appendices, would you also be willing to share screenshots of the relevant appendix pages? I’m just trying to make sure I have all the required materials for the course.
