5 weeks after car accident nose injury — persistent redness + slight ridge. Normal healing or scar treatment needed?
34M.
About 5 weeks ago I was in a car accident and my sunglasses were smashed into the bridge of my nose. The impact caused a superficial wound/abrasion in the exact area shown in the photos. Dermabond was used initially.
PHOTOS / HEALING PROGRESSION:
~1 week:
Dermabond had come off and the area was still actively healing.
~2 weeks:
The skin had closed and the redness seemed to be fading. At this point I thought it was healing normally.
~2.5 weeks:
I accidentally caused significant friction over the newly healed area. Around the same time, I also used way too much 15% azelaic acid on the spot. The area became dramatically redder afterward.
~1 month:
The acute irritation had settled somewhat, but I was left with persistent pink/red discoloration and some uneven texture.
~5 weeks / CURRENT:
It may be slightly lighter, but the redness is still very noticeable to me.
I can also physically feel a very small ridge/firm area. In certain side lighting it sometimes looks slightly indented, although the appearance of the texture changes quite a bit depending on the angle and lighting.
The skin has been completely closed for weeks. There is no drainage, open wound, fever, or apparent infection.
CURRENT CARE:
• SPF 46–50 every day
• Biocorneum silicone scar gel
• Aquaphor as needed
• 15% azelaic acid in a very small amount at night
I previously over-applied the azelaic acid and irritated the area, so now I use a very small amount and stop if it causes irritation.
The REDNESS is by far my biggest concern. My goal is to get the bridge of my nose as close to its pre-accident appearance as realistically possible.
I am scheduling an in-person appointment with my dermatologist, but I would really appreciate a physician's perspective on the following:
-
At approximately 5 weeks, does this still look consistent with an immature healing scar / post-inflammatory erythema that could fade substantially over the next several months?
-
Can a small palpable ridge still be part of normal scar remodeling at this stage, or does that raise concern for an early hypertrophic scar?
-
Is 595-nm pulsed-dye laser (PDL/Vbeam) ever appropriate this early if the wound is completely healed and the main issue is persistent vascular redness?
-
Would it generally make more sense to address the redness first and allow the scar more time to mature before considering any resurfacing for texture?
-
Is there anything I should specifically be doing — or avoiding — right now to maximize the cosmetic outcome?
I'm willing to pursue professional laser/procedural treatment if appropriate, but I also don't want to overtreat a very young scar.
Thank you for any guidance.
