Pain is real. But the intensity of pain doesn't always match the amount of tissue damage.

That's an important distinction.

You can have significant structural changes on an MRI and experience very little pain.

You can also experience substantial pain even after an injury has healed or when imaging doesn't show an obvious explanation for how much you're hurting.

Why?

Because pain isn't simply a measurement of damaged tissue.

It's a protective response produced by the nervous system using information from many different sources.

Things that can influence how we experience pain include:

• An actual injury or irritated tissue

• Inflammation

• Muscle tension and guarding

• Previous injuries

• Sleep quality

• Stress

• How long the pain has been present

• Activity levels and physical conditioning

• How sensitive the nervous system has become to a particular movement or sensation

None of this means pain is “in your head.”

It means pain is more complicated than:

Damage = pain.

That's also why treating chronic pain sometimes requires looking beyond the exact spot that hurts.

The goal isn't just to temporarily turn down pain.

It's to understand what continues to produce or amplify the pain signal and what can safely be changed.
