My 2nd lobe piercing is 3.5 years old. I had them done at a reputable place in a large city in another country so no talking to the piercer (I live in the US). I'm a side sleeper & I am blaming that, but is this a scarred piercing bump? Is there anything I can do to make it go away after all this time? The piercing is sometimes still painful and itchy. I've started using saline spray on it again 2x per day if that could help at all. I'm willing to put in the work (including trying not to sleep on this side), I just want to make sure I'm doing the right thing for whatever it is. 

I had a larger mm threadless 18g labret on it for a while and never really noticed since it covered the bump. But now with the ring it really stands out. I have only ever used implant grade titanium on this piercing as my other very healed piercing is sensitive to any other kind of metal. 

Another thing I've noticed though unsure if it's relevant - my first lobe piercings (which are 28 years old) almost feel like they go inward like an hourglass when you pinch the lobe - they're basically unnoticeable. But the new ones feel like a hard ball where the hole is if you pinch the lobe. This is for both my left and right ear.
