Police vetting mistake

Hi guys, I submitted my police vetting document yesterday but just realised now that I completely left out section ‘1.2 Name’ empty on page 1. What do I do? Do I reach out to fmhs and let them know? Can you believe it I didn’t write my name down 🤦‍♀️

Thanks
