2nd FET loss and Unexplained RPL - what do I do now?

Advice Needed!

We just had our second FET fail, and I’m completely devastated. I don’t know what to do.

I 37F and my husband 41M had 3 spontaneous pregnancies (2024/2025) that all ended in loss around 6 weeks. We went to a RE and did the basic fertility panel, semen analysis, and RPL panel (autoimmune and clotting disorders, karotype testing)—-all normal. HSG and SIS were both normal as well. I had a biopsy for chronic endometritis which came back “inconclusive” but we treated with a round of doxycycline just in case.

We did two cycles of medicated IUIs with clomid which failed, and moved to IVF. I did an egg retrieval in Feb of this year and we got 4 embryos, of which 2 were euploid (4AC and 5AC). We did two fully medicated FET cycles (extrace tablets, estrogen patch, PIO shots, progesterone suppositories). The last FET we also added baby aspirin and a short course of prednisone steroid. But both failed, no implantation, nada.

I’m at a loss for what to do. I’m doing this with insurance, with Kaiser NorCal and have been told there is a long waitlist for egg retrieval. I’m in CA where the law just changed to cover IVF so there’s a huge amount of backlog of people trying now.

Also I’ve asked about more testing, like Receptiva Dx or Emma/alice and have been told Kaiser doesn’t do those. I feel like at Kaiser they only do very standard protocols and the very minimum.

We can’t afford to do this without insurance. I could potentially change to another insurance with my employer (Sutter PPO) but that would mean starting over with a new clinic in Jan or Feb 2027.

Should I try to press for more testing? If so, what? Would you in my position stay with the same clinic, change, or get a second opinion? Try to pay for a reproductive immunologist consult?

At this point my husband and I are even wondering about adoption, but so far I’ve read that for infant adoption it can be even more expensive than IVF! Ugh.

Sorry for the novel. I’m a mess right now.
