Only droplets with pump but stream by hand expression

hoping someone has advice or tips. I’m 6 months pp and will say I feel like none of the pumps or flanges I’ve tried draw milk out well. I’ve tried anything from 12-24 mm flanges, plastic, silicone, lacteck. Medela symphony, spectra, Eufy, manual medela and I only ever get droplets. I’ve never seen a stream. I see a small stream if I hand express but never with any of the pumps, even going 30 minutes. I also only get about 2-3 oz combined, except with my first morning pump where I get 8-9oz but am super engorged by then. I am still pumping 6 times per day and was doing 8 when I was on maternity leave. I massage my breasts too during pumping, and then hand express for a few minutes after I’m done.

why is that and what am I doing wrong? I measured myself at a 15mm. But no matter what size I use, I see areola pulled in. the 12 lacteck is the most comfortable and I feel no pinching, but may be too small. I think I have elastic nipples too. Do some just not respond to pumps? if you hand express, doesn’t that take forever and how do you do both sides at once?
