My insurance is now refusing to pay 100% of my bi salp

I received a bill $7000+. I reached out before the procedure and asked if my bi salp would be covered 100%. Florida blue said yes.

now Florida blue claims only tubal ligation is covered 100%. what do I do? the dr also assured me they would code it so that the insurance would pay. under ACA guidelines, they must pay for sterilization but bi salp isn’t included unless outright stated? do I just appeal? or do I just pay?
