Forge It Anew – Ritual softlock / quest cannot be completed

I'm experiencing a progression bug during the "Forge It Anew" quest.

I repaired St. Mihai's Sword at Uriashi, went to the cave and started the ritual at the red orb. The objective correctly displayed "Perform the Ritual".

I completed all enemy waves, but after the final enemy dies, nothing happens. The red ritual effect remains active and I cannot interact with the orb to imbue the sword.

In the Journal, "Forge It Anew" is still listed as an active quest, but it has no active objective and I cannot track it. It is also not listed as completed.

I reloaded a save from before starting the ritual and completed the entire fight two times again, but exactly the same thing happened.

During the encounter, I also noticed that new waves sometimes spawn while enemies from the previous wave (for example, two dogs) are still alive. I'm not sure if this is related.
