Why was my Entry Level Performance and Conduct ADSEP paired with an RE-4 instead of RE-3?

I failed a few inspections, took a couple months too long to phase up in A school, failed to attend a few formations and overall didn't fully adapt to the military lifestyle in a timely manner. ​My legal officer told me it would likely be an RE-3 but it ended up being an RE-4.
