Looking for a pen/tablet that i can write with that will show up as a word document on my computer?

As the title says, I'm looking for something where I can write in my hand writing with a pen or perhaps stylus, on either a sheet of paper or maybe tablet? That will take the file when i am done, and put it on my computer as a word document.

I know this may be a tall order, but wondering if anyone has a set up like this.

Thanks.
