Injury report added by mistake

I was checking nurse report after vaccines apt on my baby's nhs profile that i monitor, and i was surprised to find a document+note from a week ago about some 111call reporting a knre injury, a call i never made because it never happened, i think they probably made a mistake but its still there in her notes, it's not the first time, 3weeks ago they sent me a reminder for an apt that wasnt meant for us and they were referring to the baby as he, i have a girl...

What should i do about it?
