When did you move up doses and how did you keep the titration dates straight for your doctor?

I started semaglutide about three months ago and my provider wants me to stay on each dose for four weeks before we talk about moving up. Problem is I keep losing track of exactly when I started each new dose. I had my last appointment and she asked me when I went from 0.25 to 0.5 and I literally had to scroll back through my texts to my husband trying to piece it together from when I complained about nausea.

Now I'm about to go to 1.0 and I want to actually have my dates straight this time. I also want to be able to show her my side effects at each dose level because she keeps asking and I keep shrugging.

Do you all just use a calendar? A note on your phone? I feel like I need something but I don't want to overcomplicate it. Mainly I want to know the date I started each dose, when my shots were, and what side effects showed up at which level so I can walk into the appointment and not look clueless.
