Need help in filling patta form online

Hi Friends,

Need help what to fill the details for Patta under the Ward and Block number dropdown fields so that I get my Survey number in the TSR No. dropdown field. I tried a lot of combinations but I don't get the required value. My area was under Sembiam and then moved to Madhavaram and this also causes confusion. If there is anyone who can help it would be great. I don't want to go to e-sevai maiyam or VAO/tashildar office as it causes delay and possibly bribe.
